The CSSS de la Montagne offers a broad range of services to seniors and people with decreased independence, their families and their caregivers. CSSS professionals can evaluate various areas:


  • state of health
  • nursing care required
  • home adaptation needs
  • social milieu
  • support for caregivers
  • assistance in daily life
  • psychological support
  • end-of-life care (palliative)


CSSS de la Montagne professionals can also offer advice on nutrition, exercise, fall prevention and healthy lifestyle habits in general.

The services may be provided at the CLSC or at home. Some services are also delivered in partnership with establishments in the health network or in the private sector as well as with community organizations. 

When home care services no longer meet the needs of a person with significantly decreased independence, CLSC social workers can help you make a request for a residential placement in Intermediate Resource or long-term Residence.
